How to Order Schistosome Life Cycle Stages
  Investigators requesting life cycle stages must have authorization by Dr. Glen C. McGugan, the Project Officer at PIPB, DMID, NIH, NIAID.

    • In addition, first time recipients must sign and return to NIH an Indemnification Agreement.
    Download the Idemnification agreement from the Idemnification link in the menu and return by fax or mail to:

    To order, please fill out and submit the online Requisition for Research Materials Form.

    In addition to the quantity and species of life cycle stages requested, to expedite the order please be sure to include the information found on the following check list

    * Your name
    * The name of your institution
    * Your shipping address
    * Phone and fax numbers
    * Email address
    * The name of the Principal Investigator of the laboratory that will receive the materials
    * For snail shipments, your institute’s Federal Express, UPS, or DHL account number that can be used for shipping
    * For mouse or hamster shipments, your institute’s Emery, BAX Global, or World Courier account number (see Information on Ordering Mammals for further instructions)
